Specialist Family Practitioner Team, Pre and Post Bereavement

The team covers Lincolnshire and includes qualified child and family bereavement nurse specialists, alongside a play specialist and youth worker, all working to professional standards and ethical codes. They support children and young people under 19, and their families, to understand thoughts and feelings related to illness and bereavement, providing a confidential and safe space.

Referrals
Referrals are accepted from professionals or directly from families (by telephone), with prior contact and consent required. A completed referral form must be submitted via post or email (ulth.sfpteam@nhs.net).

Criteria

  • Children with a palliative or end-of-life diagnosis

  • Young people with a parent, guardian, or sibling with a terminal illness (prognosis under one year)

  • Bereaved children and young people following the death of a parent, guardian, or sibling

Not provided
They do not offer individual support for:

  • Stillbirth or miscarriage

  • Death of extended family members

  • Adults with non-palliative or long-term conditions

  • Separation or divorce

How they work
As part of Community Children’s Nursing Services, they collaborate with other professionals. Support is offered at home, school, or in the community, Monday–Friday, 9am–5pm (excluding bank holidays).

Other services
They also provide supervision, training, debriefs, and advice for professionals, including telephone support and education on child bereavement.
